It doesn't have to have a nut on the outside. Many are short, but I often try to find long ones that sometimes have a bend in them. cracks me up that as I say this, I have polymer sheathed rubber ones now. I couldn't find good SS ones. I just check them often.

Mine don't look any different that normal ones that you can see. There is extra rubber underneath the wheel.
